The failure of the Teton Dam outside of Rexburg, ID in 1976 is a classic case. Check out the very nice powerpoint slideshow about this human-caused calamity created by Dr. J.D. Rogers at the University of Missouri-Rolla (he created the slideshow, not the calamity!):
